By scheduling an appointment, submitting payment, communicating with our office, completing forms, using our website, or otherwise interacting with Showing All 32 Mobile Dental Care, you acknowledge that you have read, understood, and agree to be bound by these Terms & Conditions, our Privacy Policy, and all applicable office policies.
Appointments are subject to provider availability, scheduling constraints, travel considerations, and clinical appropriateness.
Appointment requests are not considered confirmed until accepted by our office.
Showing All 32 Mobile Dental Care reserves the right to reschedule, modify, or cancel appointments due to weather, equipment issues, safety concerns, emergencies, staffing limitations, travel considerations, or other circumstances beyond our control.
Patients must provide a minimum of twenty-four (24) hours' notice to cancel or reschedule an appointment.
Cancellations, rescheduling requests, missed appointments, or failure to provide access to the appointment location within twenty-four (24) hours of the scheduled appointment time may result in cancellation fees, forfeiture of deposits, or other applicable charges.
Patients who are unavailable upon provider arrival may be considered a no-show.
Patients are responsible for providing a safe, clean, and appropriate environment for treatment.
Children, pets, and other individuals must be appropriately supervised during treatment.
Our providers reserve the right to discontinue or refuse treatment if conditions are deemed unsafe, unsanitary, disruptive, threatening, or otherwise unsuitable for patient care.
All treatment recommendations, diagnoses, and services are subject to clinical evaluation and professional judgment.
Not all services are appropriate for every patient or every location.
Certain procedures may require referral to a traditional dental office, specialist, hospital, or other healthcare facility.
Payment does not guarantee treatment, appointment availability, treatment eligibility, or establishment of a doctor-patient relationship.
Showing All 32 is a fee-for-service practice. In order to dispatch a dental team, payment is due prior to service. We do not participate with insurance plans, do not submit claims, and do not provide retroactive insurance billing for services rendered. By accepting treatment, patients acknowledge and agree to this policy. Patients are solely responsible for submitting any documentation to their insurance carrier and for all communications regarding reimbursement. Showing All 32 does not submit claims nor retroactively bill insurance.
Payment is due in accordance with the terms provided at the time of scheduling or treatment.
Additional fees may apply for extended travel, additional services, emergency visits, after-hours appointments, supplies, or treatment complexity.
Prices, fees, and service availability are subject to change without notice.
To the fullest extent permitted by law, Showing All 32 Mobile Dental Care, its owners, employees, contractors, and affiliates shall not be liable for indirect, incidental, consequential, special, or punitive damages arising from the use of our services.
Patients remain responsible for following all treatment recommendations, post-operative instructions, and referral recommendations.
Patient information will be handled in accordance with applicable privacy laws and office policies.
Showing All 32 Mobile Dental Care reserves the right to modify these Terms & Conditions, office policies, fees, and service offerings at any time without prior notice.
Continued use of our services constitutes acceptance of any updated Terms & Conditions.
