Notice of Privacy Practices (HIPAA)

THIS NOTICE DESCRIBES HOW MEDICAL INFORMATION ABOUT YOU MAY BE USED AND DISCLOSED AND HOW YOU CAN GET ACCESS TO THIS INFORMATION. PLEASE REVIEW IT CAREFULLY.

I. Our Commitment to Your Privacy

Showing All 32 is committed to maintaining the privacy of your protected health information (PHI). In the course of providing surgical dental care, we create records regarding your treatment and the services we provide to you. We are required by law to maintain the confidentiality of health information that identifies you.

II. How We May Use and Disclose Your PHI

We may use and disclose your PHI for the following purposes:

  • Treatment: We may disclose your PHI to other dental and medical professionals who are involved in your care (e.g., sharing radiographs or surgical reports with your primary restorative dentist).

  • Payment: We may use and disclose your PHI so that the treatment and services you receive may be billed and payment may be collected from you, an insurance company, or a third party (including host dental facilities).

  • Healthcare Operations: We may use your PHI to operate our business, ensuring that all patients receive high-quality care and to perform necessary administrative clinical tasks.

III. Disclosures in the Mobile/Contractor Setting

As a specialized surgical provider, Showing All 32 may provide services within third-party dental facilities. Your PHI may be shared with the host facility’s administrative and clinical staff as necessary for your continuity of care, scheduling, and billing coordination.

IV. Your Rights Regarding Your PHI

You have the following rights regarding the PHI we maintain about you:

  1. Right to Inspect and Copy: You have the right to inspect and obtain a copy of your dental records.

  2. Right to Amend: If you feel that the PHI we have about you is incorrect or incomplete, you may ask us to amend the information.

  3. Right to an Accounting of Disclosures: You have the right to request a list of certain disclosures we have made of your PHI.

  4. Right to Request Restrictions: You have the right to request a restriction or limitation on the PHI we use or disclose for treatment, payment, or operations.

V. Security of Digital Records

All digital imaging (CBCT/Radiographs) and clinical notes handled by Showing All 32 are stored in encrypted, HIPAA-compliant environments.

1. Independent Private Practice Disclosure

Showing All 32 operates as an independent private dental practice. All providers, including render care under a private practice model and are not employees or agents of any hospital or third-party facility, unless explicitly stated in writing.


2. Scope of Services

Services include, but are not limited to:

  • Dental implants

  • Oral surgery

  • Sedation dentistry (where permitted)

  • Comprehensive evaluations

Services may be performed in-office, at partner locations, or via mobile/portable dental services in accordance with Texas law.


3. Emergency Care Disclaimer

This is NOT an emergency facility.

If you experience:

  • Difficulty breathing or swallowing

  • Severe infection or swelling

  • Uncontrolled bleeding

  • Facial trauma

👉 Call 911 or go to the nearest emergency room immediately.


4. Financial Policy Notice

  • Payment is due prior to time of service unless otherwise agreed

  • Accepted forms may include cash, card, or approved financing

  • Deposits may be required for surgical procedures

  • Patient or Business is responsible for all fees regardless of insurance


5. No Guarantee of Results

All treatment carries risk.
No guarantees are made regarding:

  • Implant success

  • Healing outcomes

  • Aesthetic results


6. Mobile & Partner Facility Notice

When services are performed at another facility:

  • That facility operates independently

  • Showing All 32 is responsible only for care rendered by its providers

  • Facility-specific policies may apply


7. Patient Responsibility

Patients acknowledge:

  • Accuracy of medical history

  • Understanding of risks, benefits, and alternatives

  • Responsibility to follow post-operative instructions


8. Communication Consent

By engaging with our practice, you consent to communication via:

  • Phone

  • Email

  • Text (including appointment reminders)


9. Privacy (HIPAA)

Protected health information is handled in accordance with federal law.

10. Arbitration Agreement (Binding Dispute Resolution)

PLEASE READ CAREFULLY – THIS AFFECTS YOUR LEGAL RIGHTS

Any dispute, claim, or controversy arising out of or relating to dental treatment, services rendered, billing, or interactions with Showing All 32 shall be resolved exclusively through binding arbitration, rather than through litigation in court, except where prohibited by law.

  • Arbitration shall be conducted in the State of Texas

  • The arbitration will be administered by a neutral arbitrator

  • Each party shall bear its own costs and attorney’s fees unless otherwise awarded

  • The arbitrator’s decision shall be final and enforceable in any court of competent jurisdiction

By receiving treatment, the patient knowingly and voluntarily waives the right to a jury trial and agrees to arbitration as the sole method of dispute resolution.


11. Governing Law

All services and agreements are governed by the laws of the State of Texas.
